Surveys

Build surveys for families or residents and collect answers directly in Altra — no transcribing paper forms by hand.

Family surveys can be sent as a post in the app or shared as a link. Resident surveys are filled in on a tablet or phone, usually with a staff member’s help. Every answer is collated automatically and exportable to Excel.

BUILDING A SURVEY

Choose whether it’s for residents or families. For resident surveys, consider one question per page to make it easier to answer; any survey can be made anonymous.

Add questions using five types:

  1. Multiple Choice

  2. Single Choice

  3. Single Choice - smiley scale

  4. Text box

  5. Image

Mark any question as required.

FAMILY SURVEYS

Send as a post (creates a post tagging all relevant families, with the survey link in the description), or preview it and copy the unique link to share another way.

Respondents are asked for their name, email (optional) and the resident’s name, so you know whose family each response belongs to.

Using the "Post" will automatically create a post and tag all users in the post. The description will include the survey link so you can add more info to the description before sending to families.

You can also preview the Survey and share that link with relatives by email.

Family Survey answer pages will ask families for their name, email (optional) and resident name so you know exactly which answer corresponds to which resident's family.

RESIDENT SURVEYS

When creating a resident survey, the survey page will include a dropdown to select the resident that is answering the survey.

Once you have submitted an answer for one resident you can submit another answer by selecting another resident name from the dropdown.

VIEWING RESULTS

Results collate into a single view — text answers or bar charts — and you can drill into an answer to filter responses by that person, where the survey isn’t anonymous. Add your own notes before exporting to PDF, or export raw answers as CSV.

You can drill down on the data by clicking on an answer:

That way you can see filter the reply of a specific user (if not anonymous).
